USE AND DISCLOSURE OF INFORMATION COLLECTED

Parkwood Green Medical will not disclose information about any of the personal information that you provide such as your name, address, telephone number, e-mail address or credit/debit card details to any outside parties without your consent except when required by law to do so or displayed in social media without written consent.
Parkwood Green medical will only record your e-mail address if you consent us to do so. Your e-mail address will only be used or disclosed for the purpose for which you have provided it and it will not be added to a mailing list or used or disclosed for any other purpose without your consent unless you specifically request such action.

Home Visits

Some of the doctors at Parkwood Green Medical offer home visits which are generally for patients who are too ill or lack mobility to attend the medical centre. Please contact medical centre and our staff will advise you if your treating doctor does provide home visits or how to organise a home visit using the National Home Doctor Service (after hours).
The National Home Doctor Service can be contacted on 137 425.
Please enquire regarding the applicable fees. An electronic report will be sent to the practice to update your medical file.

Referrals

A consultation with a doctor is required in order for a referral to a medical specialist. Referral periods are generally for a period of 12 months and repeat referrals do require an appointment with your doctor. Where a referral to specialists or other medical professional is made, an out of pocket fee may be applied and you may contact their office for more information about applicable fees.

Test Results Policy

To protect your privacy and ensure that all results are managed in accordance with best practice, Parkwood Green Medical does not provide or discuss results over the telephone. Please ensure a follow-up appointment is made with your doctor.

After Hours Care

Remember, in the event of an emergency always call 000.
We understand that from time to time you may require medical care when our centres are closed. As part of our commitment to patient care, we have an agreement with the National Home Doctor Service who you can contact by calling 13 74 25 .

Monday to Friday Parkwood Green Medical has a duty Doctor on call after centres close. Please call your Medical One centre to access this service, note fees apply.

After 11.00 pm the National Home Doctor Service can be contacted on 13 74 25. When you phone the locum service please have the following patients details ready:
• Phone number
• Full name
• Date of birth
• Gender
• Full address
• Symptoms

The National Home Doctor Service will ask for the name of the caller, any details related to finding the address, the name of the patient’s regular doctor and which Medical One Centre you normally attend. A full report will be sent to your regular doctor at Medical One.

Repeat Scripts

It is our policy that repeat prescriptions require an appointment with your doctor to ensure that your medication and medical condition are being managed appropriately.

All prescriptions, including repeat prescriptions, are best provided face to face at consultation to ensure patient safety.

This enables your doctor to do the following:
• Review your medical record
• Confirm correct patient details, medication and dose
• Confirm if the medication is still required, if a dose adjustment is required or if any special monitoring is necessary

Your doctor may also need to consider whether a special government authority is required, or special government regulations regarding the medication or other health matters affecting decisions about the medication be provided

Provision of medication;

You are encouraged to arrange provision of future prescription/medication needs during routine consultations with your doctor.

If you are unable to make an appointment for a repeat prescription, the doctor is still required to review and maintain your medical record and will charge a fee for his or her professional time spent, and costs incurred. They may also refuse the request for a prescription without consultation.

Prescription will not generally be made by doctors other than the patient’s regular doctor or for drugs of dependence such as narcotics, benzodiazepines and other sedatives.

• Panadeine Forte, Endone, Oxycodone, Targin, MS Contin
• Diazepam/Valium, Oxazepam/Serepax, Temazepam, Stilnox
